Patient Portal

The Eye Institute of Utah offers a secure, easy-to-use online Patient Portal powered by ModMed (EMA), so you can manage your eye care anytime, from anywhere.

Through the Patient Portal, new patients can complete registration forms and required paperwork before their first visit, saving time at check-in. Established patients can securely communicate with their provider and care team using any internet-enabled device. Your personal health information is encrypted and securely stored to help protect your privacy.

Note: Your username is your email address.

Patient Portal Benefits

Managing your eye care has never been more convenient. With the ModMed Patient Portal, you can:

  • Send secure messages directly to your doctor and care team
  • Complete new patient registration and forms prior to your appointment
  • Request prescription refills and medical records online
  • View portions of your medical record and visit summaries

For your privacy, sensitive health information is not sent through regular email. Messages submitted through the portal are reviewed during normal business hours. A team member will respond within the portal or reach out if needed, typically within one business day.

To check for updates or responses, simply log in to your portal account.

How to Enroll

When you schedule your first appointment with The Eye Institute of Utah, you will automatically receive an email from our office with a secure link to activate your Patient Portal account.

Simply follow the instructions in the email to create your login credentials and complete any required forms before your visit.

If you do not see the email, please check your spam or junk folder. If you still need assistance, our team will be happy to help.

How to Access Your Patient Portal

You can access your Patient Portal anytime by visiting:

https://eiu.ema.md

From there, select “Log in as a Patient” and enter your username and password.
Note: Your username is your email address.

If you are a new patient and have not yet activated your account, you may use the activation link sent to your email after booking your first appointment.
